Good afternoon everyone. Dr. Stillman here today to talk about a really interesting paper about arthritis. And this will be of any interest to anyone with joint pain. So the paper that I wanted to talk to people about today is this paper called or titled akkermansia. Musa phila major tournament conjoint and sulfate, ameliorating or aggravating osteoarthritis. as boring as that title sounds want to unpack what it really means. So achromat Cm use an affiliate or Musa Philip is a species of gut microbiome are a species of bacteria. That’s often in your gut microbiome. It’s a commensal bacteria, which means that you’re supposed to have some of it. What’s interesting about this species is it sometimes it’s not present. I know that because I test patients microbiomes on a regular basis, and sometimes they have none of this. And what this paper is premises is that the activity of a commonly used agent for osteoarthritis in particular called conjoin sulfate may vary depending on the presence or absence of this bacteria. What they say is in Jordan sulfate, we give distinct outcomes for oae treatment, depending on akkermansia mucin affiliate presence, briefly, conjoin sulfate would ameliorate osteoarthritis, if a new set of filler is present, would aggravate it if it is absence, this has to do with complex dynamics within the microbiome. What is the take home from a paper like this, and it’s an opinion paper, it’s not actually a scientific study or experiment.

What they’re saying is that what you’re putting in your gut is going to drive different species or groups of bacteria to proliferate or to disappear. And taking a supplement, if you’ve got a certain microbiome may work to alleviate a certain condition. And in other people who’ve got a different microbiome, it may have a completely different effect, and actually worse in your condition. This is why I don’t treat you know this with that. And I don’t do this whole, you know, thing that a lot of my colleagues do, which is where they’re not testing their patients, in order to understand what’s actually going on with them. They’re just saying, Oh, you’ve got joint pain, take some turmeric, that can work. But you also find that people either may not respond, and so they’re wasting their money on the supplement. Or it may actually make them worse. And this is an example I think of some clinicians thinking critically and logically about why a supplement like enjoying sulfate has had such a mixed review or mixed results in the literature. And this is something that I see commonly people saying is, Oh, well that there’s no evidence for that, or the trials for that didn’t pan out. Well, those trials don’t take into account your biochemical individuality, they don’t take into account the fact that you have a unique microbiome. And it’s not the same as somebody else’s, who may have the same diagnosis as you. So what we really need to do is get to the bottom of what’s imbalanced in your system. And the way we do that is comprehensive laboratory testing. That can include a really wide variety of parameters. And I’m very gratified by the results that I get when I actually do that, instead of just relying on, you know, oh, we threw this drug or this supplement or did this surgery on these people who have the same diagnosis, some of them got better, some of them got worse, some of them didn’t see a benefit. When you tailor your approach to the patient and you take into account all the different factors that can you know, create or resolve disease, that’s when you really get wonderful results.
